Care Plan/EPC referrals

Patients with chronic illnesses are frequently put on a Care Plan that allows them to see several allied health providers and dentists on a limited basis. We frequently receive inquiries from patients who have been advised by these providers to seek a Care Plan from their GP. This is sometimes misleading to our patients. To be eligible, these Care Plans must be at the discretion of the doctor and the patient must have been diagnosed with a chronic disease. If you're unsure, consult your GP at your next appointment. To complete the Care Plan, a separate visit with the Registered Nurse is required. Please inform Reception when arranging an appointment.

Travel Vaccinations and Immunisations

It is advised for those going overseas to schedule a visit to a GP 6 to 10 weeks prior to your trip. Our medical centre will be able to help inform and refer you to vaccines that apply to your needs. Doctors House has the most standard vaccines available. If you have already collected a vaccine, please advise the reception, as they will need to ensure the vaccine undergoes cold chain management. For any further inquiries, please call us on 02 8103 5800.
